  Pochi-sensei’s Japanese is a bit vague(as usual), so I might be wrong here, but I would rather say that on 14th page, Yuu thanks Haru for bringing sunflowers to the hospital(which is depicted in chapter 12), with topic settled by 「ひまわりの花」. On next page, she responds with “なんだ。。。気付いていたのか”, which seem to me more of “「花を」気付いていたのか”.

  On the other hand, it can be seen as more general “thanks”, e.g. for Yuu being “saved” in previous chapter and a bit odd way of naming things, so it might be implied “not so possible to translate double meaning trick”. S:

    Hi! Yeah, somebody brought it to my attention already and it definitely was something I was confused about for a good amount of time. In the chapter where Haru is first seen in the hospital, Yuu wasn’t there, and in the chapter where she’s holding onto a single sunflower, it wasn’t explicit that she brought them in on a regular basis, so I didn’t really catch the hint.

    After some discussion with somebody else who brought this to my attention, I finally caught and understood the reference and have since made a revision. All in all, the issue boils down to the oversimplification of the two lines on page 14 by Pochi, and my lack of recognition. But anyway, it’s fixed now, so yeah :P
